It's difficult to get insurance coverage for infertility treatments, primarily because of the cost/benefit ratio. It cost tens of thousands to go through the full battery of tests, procedures, in-vitro fertilization, cryopreservation or whatever is necessary using current medical procedures. The benefits to the insurer is not so great. They spend tens of thousands to produce just one possible future customer, while leaving nature to its own costs nothing. Which makes more sense.
The insurance companies have lobbied the legislature not to mandate insurance coverage for the couples with infertility treatment needs. Fifteen states have not complied and mandate fertility insurance. These states are:

Montana
